Here is a list of common Minnesota State Moorhead financial aid forms and a brief description of their purposes.

| Authorization to Credit Account | Used to authorize Moorhead to credit any remaining funds to miscellaneous charges billed to your account, including Moorhead Bookstore charges, library fines, parking tickets, late fees, etc. |
| Degree Certification 2025-2026 |
Used to identify courses and degree program if you are pursuing a second bachelor's degree, certification, licensure, or undergoing preparatory work for a graduate program at Moorhead. |
| Dependency Support Verification 2025-2026 |
Used for to document that you provide over half the support of a dependent child while you are enrolled. |
| Federal Direct PLUS Loan 2025-2026 |
Application for the Parent Loans for Undergraduate Students (PLUS) and Grad PLUS. |
| Financial Aid Suspension | Used to appeal your financial aid suspension. |
| Identity and Statement of Educational Purpose 2025-2026 |
Used to certify you will use financial aid for educational purposes and to pay the cost of attending Moorhead. Note: Federal regulations do not permit schools to accept this form in an electronic, scanned, or any digital format. Complete the form in person at our office or submit the notarized form by mail. Please read the instructions carefully. |
